Special Needs Plans (SNPs) are a type of Medicare Advantage plan that were created to tailor their coverage to meet the needs of specific individuals. There are three different types of SNPs:

Chronic Condition Special Needs Plan (C-SNP)
C-SNPs were designed for individuals with disabling or severe chronic conditions, such as:
- Cancer
- Heart failure
- Dementia
- Autoimmune disorders
- Lung disorders
- Neurological disorders
- End-Stage Renal Disease
- And more
Dual Eligible Special Needs Plan
(D-SNP)
D-SNPs are available to those who are eligible for Medicare and Medicaid. This Special Needs Plan is considered as one of the most complex SNPs as they are tailored to meet the needs of those who have specific healthcare needs but also are not able to pay for their care.
Institutional Special Needs Plan
(I-SNP)
I-SNPs are available to those who must be in a medical institution for at least 90 days or longer. This can include skilled nursing facilities, long-term care facilities, nursing facilities, and other care centers.
Pros and Cons of Special Needs Plans
As with any plan, there are pros and cons to Special Needs Plans. Advantages of these plans can include:
- Care coordination
- Low costs
- Additional benefits not included in Original Medicare
- Prescription coverage
Disadvantages can include:
- Limited availability depending on where you live
- Referrals are needed for specialists
- Receiving care only from the plan’s provider network
